Home Health Services
Having been in the healthcare industry for over 35 years, Santa Rita understands the medical needs of those patients that need skilled home health care, and those that need some additional assistance with day to day tasks and activities of daily living. That is where Santa Rita’s Home Health Division can help! We offer skilled Nursing, Physical and Occupational therapy at home, when ordered by a physician. Our Home Health nursing and therapy staff can administer medications, change dressings, perform range of motion activities, and help create an individualized plan of care that allows patients to regain their independence and prior level of functioning, as well as have a successful transition from hospital or skilled nursing facility, to home and avoid further hospitalizations.

Private Duty Caregivers:

For those who may not need to regime of in home nursing or therapies, but still need some assistance, or for loved ones who need respite, our Caregivers are the perfect solution. We offer a variety of packages that include different daily and weekly services to make the most of the time your caregiver has with you.
Our trustworthy, reliable and dedicated caregivers go through the same extensive and thorough background checks and comprehensive training as all of our inpatient staff. They are carefully screened, references checked, CPR and first aid certified, fingerprinted, and undergo ongoing training by our licensed nursing and therapy staff in all areas of care. Additionally, all employees are bonded and insured.
Santa Rita’s Caregiving services offer flexible schedules from 1 hour of service to live-in’s, on-call staff available 24 hours a day, and a Care Manager that coordinates with family members, and your physician to create a service plan that best meets your needs.
Our Caregivers can provide assistance with:
  • Dressing
  • Grooming
  • Bathing
  • Toileting
  • Medication reminders
  • Meal preparation
  • Light housekeeping
  • Transportation
  • Companionship
With flexible schedules, competitive prices and skilled, trustworthy staff, our Caregivers can allow patients to remain safely and comfortably in their home.

Skilled Home Health:

For patients who are being discharged from the hospital or other health care facility, have a newly diagnosed condition, or have decreased in functioning, our licensed therapists, and nurses will work together, with the physician, to help patient’s get back to their prior level of functioning and successfully manage any chronic conditions. They will monitor symptoms and conditions to help avoid hospitalizations and assist in transitioning back to the comfort of a patients’ own home and assist them in living as independently as possible.
We offer a full range of home healthcare services including:

Katie has worked for Santa Rita as the Social Worker, the Admissions and Marketing Director and now as the Director of Community Services/ Home Health Administrator. She has her Masters in Social Work as well as an MBA in Healthcare Management. She enjoys being out in the community and providing people with the information and resources they need to make the right decisions for their family member or loved one. She loves getting to know every person and family that comes to Santa Rita, and is excited about the new services that Santa Rita will be able to offer the community. Katie BaransSarah Gates RN
Director of Clinical Services/Assistant Administrator
Sarah Gates is a registered nurse and received her ADN in nursing from Western New Mexico University in 2006. She is currently pursuing her BSN. During her career as a registered nurse, Sarah has gained vast experience in a post-anesthesia care unit, intensive care unit and in home health. Her passion lies with home health and believes the best way to serve a patient is in their own home. While not working, Sarah spends time with her family hiking, skiing, and enjoying good food. Sarah currently serves as the Director of Clinical Services for Santa Rita Home Health and can be reached at hhnurse@santaritacare.com
Terri YoungTerri Young
Director of Assisted Care Services
Terri came to Santa Rita after spending 10 years assisting dislocated workers and veterans back into the workforce. She has expanded on her desire to help others and feels right a home with the compassionate staff and smiling residents of Santa Rita. Terri says “Growing up in Sahuarita, it feels great to assist others right here in the community I love” She enjoys spending time with her family which includes her daughter and 99 year old grandmother. As HCR ManorCare continues to flourish, our commitment to quality of care continues to be unrelenting. We strive to provide levels of care across every service line that can serve as benchmarks for the industry. We believe our Report on Quality confirms that we have achieved excellence in a wide variety of services.
In our skilled nursing centers, nearly 90 percent of our rehabilitation patients are discharged home with the majority of the others opting to stay with us for long-term nursing services.
The average length of stay for our post-hospital patients is 34 days, and over 65 percent are discharged in less than 40 days.
More than 90 percent of rehabilitation patients rated their rehabilitation therapy as "Excellent" or "Good."
Because discharged patients are leaving the hospital sooner, they are more likely to be readmitted. HCR ManorCare is working closely with hospital referral sources to help ensure smooth transitions to our centers and to provide quality medical care and rehabilitation.
Our skilled nursing and rehabilitation centers average 18 percent rehospitalization, significantly lower than the national average. Many of our centers achieve rates lower than 10 percent.
Over the past three years, we have invested more than $100 million annually in new construction, renovation and expansion of existing facilities, equipment and information technology.
We continue to fulfill and keep our promises to hospice and home health care patients. Our hospice services integrate holistic principles with palliative disease management, expert pain and symptom control, family education and psychospiritual support for end-of-life care. Meanwhile, our home health care services help patients requiring healing from an illness, hospitalization or surgery the opportunity to recover in comfortable, familiar surroundings. 
Our Arden Courts memory care facilities tailor care specific to the unique characteristics of the individual living with dementia and provide approaches that preserve remaining abilities. Approximately 95 percent of the families of residents at Arden Courts rated their overall satisfaction as "Excellent" or "Good."
